原发性肝癌是全球发病率、致死率均居前列的消化道恶性肿瘤,超70%患者初诊时已失去根治性手术机会。经动脉介入栓塞治疗是不可切除肝癌的核心治疗手段,其中栓塞材料直接决定治疗效果与安全性。临床常用栓塞材料以碘油、常规栓塞微球为主,在中晚期肝癌治疗中应用广泛,但仍存在栓塞效果有限、靶向性不足、功能单一等问题。近年来,放射性微球、智能响应型水凝胶等新型材料快速发展,极大丰富了肝癌经动脉介入栓塞治疗的材料体系。系统综述液体栓塞剂,包括碘化油、水凝胶等;固体栓塞剂,包括载药微球、空白微球、放射性微球及水凝胶等材料的性能特点、研发进展及临床应用,为临床合理选用及新型介入栓塞材料的研发与优化提供参考。
Abstract:
Primary liver cancer (PLC) is one of the gastrointestinal malignancies with extremely high morbidity and mortality worldwide. More than 70% of patients have lost the opportunity for radical surgical resection at the initial diagnosis. Transarterial interventional embolization serves as the core therapeutic strategy for unresectable PLC, among which embolic materials directly determine the therapeutic efficacy and safety. Clinically commonly used embolic materials are dominated by iodized oil and conventional embolic microspheres, which are widely applied in the treatment of advanced PLC. Nevertheless, they still suffer from drawbacks such as limited embolization effect, insufficient targeting capability, and single functional attribute. In recent years, novel materials including radioactive microspheres and smart responsive hydrogels have undergone rapid development, greatly enriching the material system for transarterial interventional embolization of PLC. This paper systematically summarizes the performance characteristics, research progress and clinical applications of liquid embolic agents (e.g., iodized oil and hydrogels) and solid embolic agents (e.g., drug-loaded microspheres, blank microspheres, radioactive microspheres and hydrogels), aiming to provide a reference for the rational clinical selection of embolic materials as well as the research and optimization of novel interventional embolic materials.
